Payment for the initial evaluation is accepted at the time of service, via check, electronic transfer, cash or credit card.

Please note that Dr. Burgut is considered out of network for all insurance carriers but will provide you with a superbill containing all the necessary information for you to submit to your insurance company.

Dr. Burgut is not a Medicare Provider and you will not be reimbursed for any service Dr. Burgut provides.

Please note that Dr. Burgut does not supply progress notes or medical records in response to insurance companies.

 

Cancellation Policy: Cancellation is required two business days in advance. For example if your appointment is on a Monday, the latest you can cancel is your appointment time Thursday.
